Social Prescribers support patients registered within our PCN which includes Clare Guildhall, Glemsford, Long Melford and Wickhambrook surgeries by aiming to connect people to social activities, clubs and groups that are on offer in the community.

Our social prescribers are Simon Haffenden and Chelsea Warden they are available for appointments at Lavenham and Long Melford.
